To complete this agreement, you must contact our office at 1-700-645-785-02135. If payment is received without contacting our office, settlement will be pull and void. Please contact us Monday through Thursday between the hours of 8:00 a.m. to 11:00 p.m. est. and friday 8:00 a.m. to 10:00 p.m.

IF PAYMENT IS NOT RECEIVED by March 31, 2011 the offer will be withdrawn.
